Shift Details: Monday -Friday Travel to Hartford, Vernon and Windsor offices.

Hartford HealthCare Medical Group is seeking a Neurosurgery Nurse Navigator to coordinate and deliver health services within the Spine Service Line. This position involves serving as a liaison between healthcare providers, payors, and patients/families, coordinating services, providing support and education throughout diagnosis, treatment, and survivorship. Other responsibilities include outcomes research, process improvement, and serving as a resource for the neurosurgery community and the public. Travel to Hartford, Vernon, and Windsor offices is required.

Qualifications

  • Licensed as a professional RN in the state of Connecticut
  • BSN required
  • Minimum of 5 years nursing experience
  • Experience within Neurosurgery Service Line or multispecialty environment preferred
  • EPIC experience
