Boulder Community health is seeking a Pathways Therapist who functions as a clinical employee that serves the needs of patients in Behavioral Health Services, and is responsible for providing psychotherapy and care management services.
The Pathways Program is a nature-based Intensive Outpatient (IOP) and Partial Hospital program (PHP). Therapy takes place on the trails around Boulder and at BCH. LCSW, LPC preferred. Knowledge of wilderness therapy would be helpful. This role includes group, individual and family therapy. Candidates will need Wilderness First Aid Certification or be willing to get this certification within 90 days from date of hire.

Qualifications
• Academic background in the area of social work, nursing, counseling, psychology, or related field with a minimum of master's degree as required for licensure eligibility.
• Must be licensed LPC/LCSW or eligible for licensure.
• Colorado LPC or LCSW Licensure required within one year of hire.

Responsibilities
• Assist in admission and orientation of patients.
• Review initial assessment(s) and assist in completion.
• Conduct patient assessment evaluation and communication to the attending M.D. and treatment team.
• Provide leadership in completion of the Master Treatment Plan and ongoing treatment goals.
• Develop, coordinate and complete discharge planning.
• Facilitate patient access to needed resources and aid in the development of alternatives as necessary to meet identified needs.
• Maintain collateral contacts with family or support systems, outside agencies and professionals as necessary.
• Attend and participate in team clinical supervision, team meeting and treatment planning.
• Meet with patients (weekly) to assess and evaluate status and discuss measurable treatment goals.
• Actively involve patient and family in the planning and implementation of care as appropriate.
• • Maintain communication with the patient's referring therapist, psychiatrist and/or primary care physician.
• Management of patient milieu.
• Participate in Program QA/SQI as assigned by Program Manager.
• Ability to assess, treat, care for patients age adolescent to geriatric.
• Completion of daily charges.
