We are searching for a devoted RN case manager to coordinate and advocate for our patients' care & create an appropriate discharge plan.
The RN case manager will be required to review patients' charts, and to meet
with families and healthcare providers to establish the most suitable course of action.
The Case Manager should remain in contact with patients and their families during the hospitalization stay.
Qualifications
Job Requirements:
- Must maintain current Texas RN License
- BLS Required.
- 2 years prior clinical experience, preferably in inpatient Behavioral Health, or home health case-management environment
- ADN required
