We are currently recruiting for an Integrated Care APRN for our school based health center located in Bridgeport, CT. This is a 35 hour per week position & this role requires participation in our Saturday rotation ( 9:00 am-1:00 pm ) and our telephonic call rotation.

POSITION SUMMARY

The Integrated Care Nurse Practitioner / Advanced Practice Registered Nurse is delegated the authority by their supervising physician to perform physical examinations and order diagnostic testing including but not limited to laboratory tests, diagnostic imaging tests; to make clinical assessments, and implement a plan of treatment, to include, but not limited to, the prescribing of medication as so granted by Connecticut statutes.

FUNCTIONS & RESPONSIBILITIES

1. Obtain complete medical history and physical data on patients.

2. Interpret and integrate data to determine appropriate diagnostic and therapeutic procedures as needed.

3. Perform complete physical exam and records findings in electronic medical record. Collaborates with physicians as needed in managing acute and long-term medical needs of the patients.

4. Order appropriate laboratory and diagnostic procedures.

5. Synthesize data to determine preliminary diagnosis and therapeutic plan utilizing principles of prevention.

6. Administer medications and injections as needed.

7. Interview and advise patients regarding health and illness prevention. Recommends community resources to meet patient and family needs.

8. Instruct patient and family regarding medications and treatment instructions. Provides patient education.

9. Maintain and review patients’ records, charts, and other pertinent information. Posts tests and examination results.

10. Triage patient telephone calls and provides consultation, and appropriately refers patients to additional needed services.

QUALITY ASSURANCE, SUPERVISION & MONITORING

Optimus Health Care, Inc. will provide quality assurance protocols and adequate monitoring of the tasks so delegated by any or all the following:

1. Onsite supervision and consultation between the non-physician practitioner and an available physician with the department.

2. In cases where there is no onsite supervision, there will always be electronic means of contact and consultation between the physician, and the Integrated Care Nurse Practitioner / Advanced Practice Registered Nurse when so needed.

3. When the supervising physician is not available for consultation, this authority may be delegated to other physician members and will be communicated to the Integrated Care Nurse Practitioner when needed.

JOB QUALIFICATIONS/REQUIREMENTS

EDUCATION: Graduate of an accredited school of nursing and successful completion of an approved Nurse Practioner program.

EXPERIENCE: One to three years’ experience in pediatrics and / or adult medicine. Familiarity with telemedicine and prior experience a plus.

LANGUAGE SKILLS: Bi-lingual skills in Spanish preferred

KNOWLEDGE, SKILLS, AND ABILITIES: Knowledge of professional nursing theory, practice, and medical care to give and evaluate patient care. Knowledge of organizational policies, regulations and procedures to administer patient care. Knowledge of medical equipment and instruments to administer care. Knowledge of common safety hazards and precautions to establish a safe work environment. Skill in applying and modifying the principles, methods, and techniques of professional nursing to provide ongoing patient care. Skill in taking medical history to assess medical condition and interpret findings. Skill in preparing and maintaining records, writing reports, and responding to correspondence using current electronic media. Skill in developing and maintaining quality control/assurance standards. Ability to react calmly and effectively in emergency situations. Ability to interpret, adapt and apply guidelines and procedures. Ability to communicate clearly.

LICENSURE / CERTIFICATION: Licensed to practice as a Nurse Practitioner / Advanced Practice Registered Nurse in the State of Connecticut. Board certified in Family Practice or eligible for certification.
