RECOVERY SUPPORT NAVIGATOR-ORANGE TEAM

In this role you will:
  • Monitor patient progress via patient level of functioning, adherence to treatment plan, recovery/relapse process and/or service needs.
  • Assist with medical insurance application process (guide prospective enrollees through completing application, obtaining documents, etc).
  • Ensure that patients have and maintain medical insurance coverage for engagement in care.
  • Assist with scheduling outpatient visits and follow-up as needed.
  • Provide education, counsel, and support clients. Use a variety of culturally, linguistically and educationally appropriate strategies, in a variety of settings. Coordinates with Interpreter Services, as needed.
  • Collaborate and coordinate with health care team members, including social work, to promote positive prevention, and harm reduction.
  • Compliance and Contract reporting.
  • Provide other administrative and special project assistance as required or assigned by Program Manager.
The successful applicant will offer:
  • Bachelors or associate degree (or equivalent combination of formal education and experience in community health) plus a minimum of 1 - 2 years in healthcare is required.
  • A Bachelor of Science in Public Health, Biology, Psychology, Education, Social Work or an equivalent degree preferred.
  • May waive experience requirement for candidates with bachelor’s degree or higher.
  • At least 2+ years of experience working in a health care setting required; preferably with individuals with substance use disorders preferred.
