We are seeking an experienced and dedicated Full or Part-Time Case Manager to oversee the care team of State Tested Nursing Assistants (STNAs) and Medical Assistants providing in-home care to a client with a traumatic brain injury (TBI). The Case Manager will ensure that care plans are effectively implemented, coordinate services, and provide support to both the patient, family, and the care team. This is an onsite role in Avon, Ohio.
Key Responsibilities:
- Care Coordination:
- Develop, implement, and regularly review individualized care plans for the TBI patient in collaboration with healthcare professionals, the patient's family, and the care team.
- Schedule and coordinate care team members, ensuring coverage and continuity of care.
- Serve as the primary point of contact for the patient and their family regarding care-related concerns and updates.
- Supervision and Support:
- Oversee the performance and activities of STNAs and Medical Assistants, providing guidance and support as needed.
- Conduct regular assessments and evaluations of care team performance, providing feedback and training to ensure high-quality care.
- Address any issues or concerns related to the care team's performance or patient care.
- Medical and Administrative Duties:
- Monitor and document the patient's progress, including changes in condition, response to treatment, and any new medical needs.
- Ensure that all care documentation is accurate, timely, and compliant with healthcare regulations and agency policies.
- Coordinate with external healthcare providers, including physicians, therapists, and other specialists, to ensure integrated care.
- Quality Assurance:
- Implement and adhere to quality assurance protocols and best practices to ensure the patient's care is safe and effective.
- Review and update care plans and protocols as necessary to adapt to the patient's evolving needs.
- Handle any complaints or concerns from the patient, family, or care team promptly and professionally.
- Education and Training:
- Provide ongoing education and training to the care team on relevant topics, including TBI-specific care needs, safety protocols, and any new procedures.
- Keep abreast of developments in TBI care and home care practices to ensure the care team is knowledgeable and up-to-date.
- Emergency Response:
- Develop and review emergency care plans, ensuring all team members are aware of procedures and protocols.
- Respond to emergencies effectively, providing guidance and support to the care team and coordinating with emergency services if needed.
Qualifications:
- Education: Bachelor's degree in Nursing, Social Work, Healthcare Management, or a related field preferred. Relevant certifications or licensure (e.g., RN, LSW) are advantageous.
- Experience: Minimum of 2-3 years of experience in case management or related roles, with a preference for experience in home care or working with patients with traumatic brain injuries.
- Skills:
- Strong organizational and multitasking skills with the ability to manage and prioritize multiple responsibilities.
- Excellent communication and interpersonal skills, with a proven ability to work effectively with patients, families, and care team members.
- Proficient in care coordination, medical documentation, and regulatory compliance.
- Knowledge:
- Familiarity with TBI and its management, including common challenges and care strategies.
- Understanding of home care regulations and standards.
- Background Check: Successful completion of a criminal background check and other required screenings.
- Transportation: Reliable transportation with a valid driver's license and proof of insurance, if travel is required.
