PAY RANGE: $46.51 to $63.18

Established in 2023, CommuniCare+OLE is the result of a union of two health centers with a deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare+OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs.

JOB SUMMARY: The Registered Nurse (RN) is responsible for providing quality primary health care to patients under the direction of staff Providers and within their scope of practice. In collaboration with staff Providers, conduct assessments and develop and implement a care plan for the patients of CommuniCare+OLE.

Ability to collaborate with mutual providers of treatment in all current clinical settingsAbility to prioritize and multi-task and have strong time management skillsKnowledge of medications
DUTIES AND RESPONSIBILITIES

Travel between Napa sites (North Napa Campus, South Napa Campus) and Solano sites (East Fairfield and West Fairfield)

Performs the following patient care according to clinic protocol and consult with staff Providers as needed; physical evaluations/assessments, wound care, insulin titration, anticoagulant management, catherization, port flushes, and IV insertion and management
Provides health care education and handouts on a variety of issuesAdministers medications and treatment regimensPerforms medication reviews, clarify and classify, and submit refill authorizations per protocolRefers patients to relevant in-house and community services as appropriatePrioritizes patient care throughout the dayResponds to messages and relay information to patients and other external sources as neededProvides acute triage over the phone or with walk in patientsDirects and coordinate patient care activities of support staff as allowed under the Nurse Practice ActReviews labs and advises patients of results per protocolsTrains and supervises students as neededAssists in the ongoing development and implementation of performance-improvement activities such as quality assurance and peer reviewDocuments all appropriate information in the electronic health record systemProvides patient care based on practice guidelines, standards of care, and federal/state laws and regulationsPerforms other duties as assigned
EDUCATION, EXPERIENCE, TRAINING
  1. Associate’s Degree or higher in Nursing required.
  2. Current California Registered Nurse License required.
  3. Must certify and remain current in CPR certification.
  4. Experience in a community healthcare center preferred.
  5. Training in motivational interviewing preferred.
  6. Strong verbal and written communication skills; Bilingual (English/Spanish) language preferred.
  7. Strong interpersonal and customer service skills required; must have ability to work with a diverse population.
  8. Strong computer and data entry skills required; electronic health record system preferred.
  9. Strong analytical and problem-solving skills required.
