Your day.
As a Licensed Clinical Social Worker, you need to know how to:
- Conduct psychosocial assessment interviews with patients to approve or deny readiness for bariatric surgery
- Provide triage, assessment, and counseling for patients as identified as needing behavioral health intervention
- Develop and implement treatment plans for stabilization and behavioral modification, mobilization and disposition appropriate for patient care.
- Function as part of a multidisciplinary team to develop patient care plans
- Provide services using accepted psychotherapeutic models and interventions
- Provide consultation to primary care providers as needed and collaborate with staff and providers on treatment plans.
- Consult and act as liaison with all other members of the treatment team to mobilize patient
- Provide educational information to patients and their families
- Provide aftercare planning
- Participate in patient-centered departmental and inter-disciplinary meetings as well as non-departmental meetings and ongoing hospital committees as requested.
- Maintain clinical skills through continuing education.
- Complete all required competencies and maintain all health standards as required by hospital in a timely manner

- Master's Degree in Social Work
- Licensed Clinical Social Worker in the State of Colorado (LCSW)
- Knowledge and experience with managed care
- Spanish proficiency
- Previous experience in integrated healthcare, bariatric or primary care
- Experience working with and sensitivity toward special populations, including those with limited income, limited education, and non-English speaking individuals
- Experience with PC software applications in word processing, Epic, Google applications
