JOB SUMMARY
Under the supervision of the Co-occurring Coordinator and Sr. Director of Integrated Services, the Co-occurring Clinician functions as part of the treatment team to provide mental health counseling services, screenings/ assessments, psychotherapy, case management, and care coordination functions with Eva's Village's Psychiatrist and/or APN. The Co-occurring Clinician will work very closely with Eva's Village Integrated Service Team (i.e.- Clinical, Medical, Residential), and any other relevant staff to ensure care coordination and service linkages for clients.
JOB DUTIES
1. Completes comprehensive screenings and assessments of clients if indicated and ensures treatment of clients with co-occurring disorders is provided by appropriately trained and qualified clinical staff (N.J.A.C.-10:161B-1.8(d)5 .
2. Completes detailed bio-psychosocial assessments and/or evaluation of client's history of psychological and/or psychiatric treatment (i.e.-which shall include, but not limited to previous admissions to psychiatric facilities, history of suicidal/homicidal ideation and attempts, outpatient psychiatric treatment, psychotropic medications, etc.) N.J.A.C.- 10:161b-9.1 (b)4.
3. Completes DSM-5- TR diagnoses, Individualized Service Plans (ISP), level of care recommendations and service pre-authorizations as indicated.
4. Complete client service plan by the fifth session or within 60 days of the first face-to-face visit, whichever occurs first, except when documented that clinical circumstances dictate that a different time frame is in the client's best interests (N.J.A.C.-10:37E-2.4)
5. Provide service plan review at significant decision points in each client's course of treatment and the review shall be documented in either the progress notes or treatment plan revision. Significant decision points shall include, but need not be limited to, the transfer or discharge of a client, changes in medication, and any significant change in the client's condition or situation, including, at a minimum, adverse reactions to medications (N.J.A.C.-10:37E-2.4)
6. Provide review of the service plan together with the client at least every three months for
the first year of treatment and at least every six months thereafter.
7. Provides Co-Occurring individual counseling to clients.
8. Provides Co-Occurring group counseling to clients.
9. Provides educational/didactic groups on mental health and recovery related issues.
10. Provide detailed daily progress notes for all individual, group, and/or family counseling services in clients electronic health record (EHR) at the close of business day.
11. Provide case management duties (i.e.-makes referrals and coordinates scheduling of clients for evaluations and follow-up appointments with Eva's Village's psychiatrists and/or apn or other community psychiatrists/behavioral health providers).
12. Participates in clinical staff meetings within the agency.
13. Maintains effective intra and inter communication within departments.
14. Participates in case conference with multi-disciplinary treatment team (MDT) members as clinically needed, to determine the best course of treatment for clients and to address all the client's needs as per the treatment plan.
15. Maintains client Electronic Health Record (HER) and assures up to date client documentation for mental health services provided.
