Responsibilities: The Speech Language Pathologist will deliver outpatient rehabilitation services primarily in an in-home setting for the older adult population. This could involve working in a patient's private residence, assisted living facility, or independent living space. Our model allows the Speech Language Pathologist to assess and treat within the most functional environment—the home. Responsibilities include:

  1. Supervising clinical fellows, temporary licensees, students, and support personnel as appropriate.
  2. Adhering to treatment schedules and timely clinical documentation.
  3. Attending and contributing to patient care conferences and related meetings.
  4. Making referrals to other professionals to comprehensively address patients' needs.
  5. Consulting with the rehabilitation team and physicians to tailor therapy regimens.
  6. Adapting programs according to individual patient needs.
  7. Ensuring a safe environment by identifying and mitigating hazards.
  8. Ordering and maintaining necessary supplies and equipment.
  9. Delivering high-quality care with compassion, recognizing individual patient rights.
  10. Evaluating and diagnosing patients to design effective treatment plans.
  11. Implementing therapeutic techniques across various areas, including:
  • cognitive-communication
  • speech intelligibility
  • swallowing disorders
  • memory
  • functional communication
Coordinating the acquisition and instruction for adaptive communication devices. Identifying hearing deficits and facilitating appropriate referrals. Selecting patient-centered activities that align with individual capabilities and interests. Instructing patients in therapeutic techniques and evaluating progress. Modifying treatment plans based on routine reexaminations. Collaborating with an interdisciplinary team to coordinate home and community care.

Qualifications:

  • Must possess a valid Speech Language Pathologist license in the state(s) of practice or provide proof of eligibility.
  • Must have or be eligible for a Certificate of Clinical Competence from the American Speech-Language-Hearing Association (ASHA) or be eligible as an ASHA Clinical Fellow.
  • Must possess a valid driver's license or reliable transportation to patient residences.
