Shift: Monday - Friday 8:00 A.M. - 5:00 P.M.

Key Responsibilities:

  • Have knowledge of the Clients Rights and ensure the client's rights are not violated.

  • Provide services at times requested, including weekend and evening hours.

  • Participate in clinical team meetings to identify client strengths and needs related to recovery plan.

  • Assess clients for housing needs, strengths, and resources.

  • Arrange and refer for services and resources, advocating to obtain entitled services.

  • Coordinate and facilitate housing search and placement, and coach for success.

  • Have a working knowledge of community twelve-step recovery resources.

  • Monitor services by external providers to ensure communication and coordination.

  • Provide experiential training in using self-advocacy skills and resource acquisitions.

  • Provide information and education in accordance with rehabilitation plan and discharge plan.

  • May participate in community events, media and committees as deemed necessary.

  • Function in accordance with organization's policies and procedures.

  • Participate in and contribute to staff development activities.

  • Participate in all required meetings.

  • Other duties as assigned by leadership.

Education and/or Experience Qualifications:

  • Bachelor's Degree from an accredited college or university in social work, psychology, nursing or a closely related field.

  • Equivalent experience may be substituted on the basis of one year for each year of required educational training with a base education of a High School Diploma or GED.
