Job Summary:

Manages the overall operations for nursing services and patient care at two or more facilities, including but not limited to financial management, quality assurance and patient care, safety, risk management, employee management and satisfaction, quality indicators, facility maintenance, and other required duties that may arise during the general operations of the facility. This position assures the safe, therapeutic nursing management of the End Stage Renal Disease (ESRD) patient.

Financial Management
  • Drive clinical and operational outcomes
  • Ensure that compliance and audit requirements are met
  • Assist manager to achieve labor hour goals as well as budget targets for operational goals through the use and/or implementation of the appropriate tools
  • Develop improvement plans for outliers
  • Participate in budget formulation and review concerning personnel and scheduling needs with patient load within established labor hour goals
  • Support market development and growth
  • Develop strategic growth initiatives as needed
  • Support the integration process of acquisitions within the designated market
Peritoneal Dialysis Program Management
  • Oversee nursing and clinical services for patient treatment and ensure that appropriate adjustments in treatment plans are developed, implemented, and meet compliance with state and federal governing agencies
  • Develop facility action plans and ensure resolution of issues
  • Participates in development and implementation of PD policies, procedures, and resources to ensure safe, appropriate treatment of patients Maintain open communications with physicians, nurse practitioners and other healthcare professionals regarding patient care
  • Develop rapport with patients, families, vendors and manage patient concerns timely and appropriately
Patient Care Management
  • Supervises and performs direct patient care including but not limited to taking and monitoring vital signs, initiation and termination of dialysis, documentation, physical assessment of patients and patient treatments, taking and transcription of physician's orders, drawing and documentation of ordered laboratory work
  • Notes signs of trauma, patients' general physical and mental condition, reviews dialysis orders, documents observations in patient medical records
  • Analyzes patient assessments, laboratory data and, when concerns are noted, brings issues to the attention of the physician
  • Communicates all emergencies, patient life-threatening complications and related incidents to supervisor, Medical Director, Director of Clinical Operations, Facility or Area Facility Administrator
  • Assesses patient and family learning readiness and provides appropriate information on renal disease, prescribed dialysis treatment, and medical regimen and its impact on the patient's state of health and well-being
  • Provides peritoneal dialysis therapy within the scope of practice and according to the plan of care; recommends a dialysis plan for ultra-filtration; complies with all safety measures, monitors patient vital signs applicable to ESRD patients; administers medications per physician instruction if allowed by state agencies, fluid therapy and blood products or other appropriate treatments as needed; documents activity appropriately
  • Instruct patients on pre- and post-dialysis care
  • Administers patient quality care in a considerate, respectful manner
  • Participates in the development and implementation of the patient's short and long-term plan of care and monthly progress notes
  • Is a resource for new RNs
  • Provides unsupervised patient care
  • Supervises day-to-day work of Patient Care Technicians if utilized in the PD Program
  • Demonstrates the ability to transfer skill set to other clinics and works at other clinics when required
  • Meets qualification for Charge Nurse; CQI program and outcome management
  • PD Program Quality Assurance Program Initiative (QAPI) leader and driver for improvement
  • May function as a Preceptor on a regular basis
  • Is a Pre-End Stage Renal Disease Patient Education Program Captain
  • Is a resource for colleagues with less experience
  • Directs the work of other staff when indicated
Required License(s)/Certification(s):
  • A Licensed Registered Nurse credentialed by the State Board of Nursing in the State where the site is located or licensed in a compact licensure state provides a multistate license allowing nurses to practice with patients across state lines.
  • Must obtain CPR/BLS certification within first 60 days if not already certified
Education/Experience/Qualification:
  • Degree from an accredited school of nursing required; Bachelor's Degree preferred
  • Experience in Peritoneal Dialysis required
  • Current driver's license, reliable transportation, and vehicle insurance in state of residency
  • CNN or CDN preferred
  • Experience in case management, home health, disease management, or hospice preferred
  • Must have demonstrated record of commitment to safety and dedication to excellence.
  • Must possess outstanding communication and interpersonal skills. Must be able to communicate professionally and effectively with other staff members, patients, and physicians.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to speak effectively with clients and fellow staff members.
  • Must have computer skills including data entry, navigation around software and basic troubleshooting skills
