Bilingual Case Manager (Spanish/English)  Long Term Foster Care (LTFC) Reports to: LTFC Coordinator of Case Management Services Hours : Full-Time position   Holy Family Institute (HFI) is a 125-year-old Catholic organization with an impeccable history of serving children and families in the Pittsburgh Region. We are one of Pittsburgh’s largest social service organizations. We are an independent organization guided by Catholic Social Teachings and serving people of all faiths and backgrounds. The Case Manager is responsible for coordinating case management services. The Case Manager’s tasks shall include development of intake assessment, individualized care plan, family reunification, permanency planning, post-18 planning, transition to adulthood, independent living support, and discharge planning. The Case Manager shall also ensure that all services provided to each child are properly documented in each child’s case file. Final approval for hire will be done by the Project Officer of the Office of Refugee and Resettlement which is a sub-agency of the Department of Health and Human Services of the federal government. Works with a multifunctional team to maintain well-being and safety of minors and to ensure that services are provided. Document progress that families and sponsors make toward risk reduction, achievement of service goals and positive case outcomes. Utilizes the assistance of Case Aides to achieve program goals. A willingness to travel when needed to support case-related activities, youth transitions, reunification, discharge, transfers, placements, or other program-related needs. Able to work holidays when needed, as well as participate in an on-call rotation. Models and assists in the implementation of the positive behavior management techniques, such as HART Model and Trauma-Informed Care Ensure confidentiality in all program operations and service population. This includes but is not limited to sexual abuse in a prison, jail, holding facility, community confinement facility, juvenile facility, other institution (as defined in 42 U.1997), or care provider facility; Completes other duties as assigned by supervisor Bachelor's degree in behavioral sciences, human services or social services fields including psychology, sociology, social work or counseling.  Must possess exceptional time management and detail-orientation skills. Child welfare and/or case management experience is strongly encouraged. Candidate must be fluent in both English and Spanish (read, write, and speak). Must be proficient in Microsoft Word, the Internet, and e-mail.  Must have a valid driver’s license and be able to drive a car or van.
