If that sounds like an organization you want to be a part of, we would love to have you.ROLE OVERVIEW and PURPOSEThe Occupational Therapist works to support the mission and vision of Neighborhood Healthcare by providing occupational therapy services for patients to remain independent and live safely in their communities.This role will design and develop patient care plans based on mobility assessments.In addition, this role will supervise the support team to ensure patient care plans are carried out correctly to increase success rates for patients.Hybrid role: 30 hours on-site/10 hours remote.RESPONSIBILITIESDevelops and implements occupational therapy programs for patients accessing day servicesPerforms assessments of patients to determine physical abilities and mobilityDevelops and implements individualized plans of care to decrease or eliminate disability, increase, or maintain capability for independence, and develop function to a maximum levelMonitors qualified support staff activities to ensure individual plans of care are followed appropriatelyGuides patients and caregivers in the use of adaptive devices and therapeutics and creative and self-care activities for improving functionEvaluates the effectiveness of treatment plans and modifies goals, as neededConducts home, hospital, and other site visits as indicated by IDT and Center ManagerRecords and signs treatment plans in the patient's health recordRecords, signs, and dates quarterly progress notes; including reviews and initials all evaluations and discharge summariesEnsures services provided are in a therapeutic setting and conducive to restoring dignity and self-esteem to patientsManages required equipment and supplies for the occupational therapy program to ensure they are available for patients; order equipment as neededFollows policies which define the occupational therapy responsibilities as a resource to support staffKeeps abreast of all regulations and standards to ensure compliance with orders or directives issued by duly constituted government and regulatory agencies (PACE, CMS, DHCS)Participates in training programs for staff and caregiversEnhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshopsEDUCATION/EXPERIENCEMaster's degree in occupational therapy and two years' occupational therapy experience requiredValid CA Occupational Therapist license in good standing requiredOne year experience working with a frail or elderly population requiredADDITIONAL QUALIFICATIONS (Knowledge, Skills and Abilities)Excellent verbal and written communication skills, including superior composition, typing and proofreading skillsAbility to interpret a variety of instructions in written, oral, diagram, or schedule formKnowledgeable about and experience with principles and methods for treatment and rehabilitation of functional impairmentsAbility to establish and maintain therapeutic relationships and to participate in a multi-cultural, multidisciplinary teamKnowledge and understanding of the Older Americans Act, Americans with Disabilities Act, and Area Agencies on AgingAbility to apply ingenuity and imagination in adapting activities to individual needsAbility to successfully manage multiple tasks simultaneouslyExcellent planning and organizational abilityAbility to work as part of a team as well as independentlyAbility to work with highly confidential information in a professional and ethical mannerPhysical RequirementsAbility to lift/carry 25 lbs/weightAbility to stand for long periods of timeNeighborhood Healthcare offers a generous benefit plan that includes:Partially company paid Medical, Dental, and Vision Plans.
