Job description:
SLP working primarily with outpatient pediatrics, outpatient adults and some subacute inpatient coverage as needed & modified barium swallow studies.

Position Purpose:
To provide diagnostic and therapeutic services, patient/family education, and appropriate referrals to patients exhibiting a speech, language, or swallowing disorder. Maintain quality care for all patients through a coordinated, comprehensive, multidisciplinary approach; and providing assistance where communication barriers exist due to speech/language disabilities to enable each individual to reach his or her maximum level of independence.

Essential Functions and Basic Duties
Position Specific Performance Expectations:
Coordinates and directs patient care services to patients of all ages to ensure that patient needs are met. Services include patient evaluation/ assessment, establishing and implementing a plan of care, assessing effectiveness of and modifying treatment to ensure goal achievement, and educating patients/caregivers.
Interacts professionally with the patient and/or family/caregiver and involves them in the plan of care.
Demonstrates the ability to apply therapeutic principles and practices based on current evidence and knowledge, appropriate clinical judgment, and patient preference.

Practices in a safe manner that minimizes the risk to patient, self, and others
Produces quality documentation in a timely manner to support provision of care, according to current standards that are based on professional association and regulatory guidelines.

Submits appropriate & accurate documentation and timely billing/coding information for services rendered.

This includes seeking opportunities to gain new knowledge and skills and utilizing these newly acquired skills in the provision of care and for growth and development of the department and hospital.

School-based speech language pathologists (in addition to above expectations): Collaborate with the school-based team to help determine and carryout Individualized Education Programs (IEPs); identify a student's annual goals and determine the services, supports, modifications, and accommodations that are required for the student to achieve them.
Home health speech language pathologists (in addition to above expectations): Provide in-home speech therapy services to homebound patients and work in collaboration with the home health interdisciplinary team.
Performs other duties as assigned.

Education/Licensure/Certification:

Experience:
Current Certificate of Clinical Competence granted by the American Speech Language Hearing Association, in Speech Language Pathology (CCC-SLP) preferred.
