Position Summary:Care Hawaii is a behavioral health care organization seeking a compassionate anddependable Team Lead Housing Specialist to support the daily functions of CAREsprovision of services. CARE Hawaii, Inc. contracts with multiple stakeholders to provide anarray of services. The Team Lead Housing Specialist is a member of a multidisciplinaryteam, responsible for organizing, coordinating, monitoring all contract operations underthe direction of the Director.The primary duties of the Team Lead Housing Specialist is to oversee the daily functionsof the program, maintaining and requesting of billing authorizations, supervision ofemployee activities and timekeeping, education and training of employees, qualityassurance reporting, and meeting all contractual requirements and best practicesguidelines. This member may work in collaboration with other providers and any othermembers of the individuals circle of support.Position Qualifications:Minimum Education: Bachelors/masters degree, preferred field experience workingwith homeless or transitional housing individuals. (Field experience may includecommunity outreach; locating individuals on the street; completing homelessassessments Vulnerability Index-Service prioritization Decision Assistance Tool(VISPDT); finding short and long-term housing; assisting individuals to apply fordocuments, benefits, and housing.)Minimum Experience: 1-year field experience with a homeless or transitional housingagency, or bachelors degree in a related field and similar field experience.Computer proficiency in various software. Preferred experience in Clinicalapplications for mental health, housing laws, knowledge of landlord/tenant rights,andRequired Certification/Registration:o CPR and First Aido TBo Background Checko CPIo Valid Drivers Licenseo Vehicle: Current and Valid DocumentationJob CompetenciesRequired Skills: Prior supervisory experience, leadership ability, demonstration inexcellence and interpersonal skills to effectively work with all levels of personnel andwith other agencies. Must know thebasics of ethics, HIPPA standards, and problem solving. Must display sound judgment, agood work ethic, and personal integrity. Must have knowledge of housing resources,2CARE Hawaii, Inc.Team Lead - Housing Specialistissues, and rental housing market in Hawaii. Ability to relate effectively to individuals witha mental illness, substance abuse issues, co-occurring disorders, and homelessness.Technical Skills: Basic use of a computer and/or tablet, Microsoft Word Suite, faxmachine, telephone, and text and chat features on the telephone or computer.Working Environment: May be in modern well-lighted, air-conditioned work areasand/or in community-based settings; provides outreach in the community.Indoor/outdoor, open windows or air-conditioned environment; may require sometravel, over-time, weekend, and holiday work, working with other Federal and/or Stateaccounting bodies, noise level in the environment may be moderate to noisy. Exposureto infectious diseases is a relatively common risk.Special Demands: Ability to respond effectively to the varied needs and demands ofmultidisciplinary team members. Have access to helpful, adequate, competent, andcontinuous supports and services. Harnesses the active involvement of team membersin taking ownership of aspects of program development, including operations, recordkeeping, staff/client education, treatment planning, and quality assurance activities.Consumers Served: CARE Hawaii will provide services to any who meet the criteria ofthe program. The treatment of individuals should reflect an understanding of thedevelopmental needs and range of treatment needs for each client.Physical Demands:1. Sit for long periods of time, use of proper ergonomics, the ability to focus on acomputer and/or tablet for long periods of time.2. Ability to assist with the high volume of calls; ability to communicate effectivelyboth verbally and in writing is essential.3. Ability to follow instructions and motivation to refer to various rehabilitativegroups. Frequent reading of printed materials is essential.4. Listening skills necessary to effectively respond to consumers.5. Ability to identify and support the daily interactions and to deal with eachsituation in a stress-free manner.Duties and Responsibilities:Essential FunctionsDirects the day-to-day operations of the team, including scheduling, staffmeetings, planning and coordination of treatment activitiesComply with policies and procedures regarding authorization requests, andcompensation & payment schedule, per contractTrack, update, and maintain all authorizations, and billing information in therequired databaseMonitor and track each members medical insurance status to ensure there is nolapse in coverageWork collaboratively with fiscal billing specialist to process authorizations and toplan for and communicate changes related to the authorization processEnsure program services are provided to consumers during the day and eveningoffice hours between 8:00am and 9:00pm. Service hours shall be provided seven(7) days a week including holidays and based on program needs3CARE Hawaii, Inc.Team Lead - Housing SpecialistJob DescriptionEffectively communicate authorization end dates and follow up with thedischarge planning process and implementationCross checking authorizations, billing, and payment to ensure there are noinaccuraciesSubmit discharges according to contract requirementsEnsure that the consumer to staff ratio is maintained so that all consumers shallbe provided with the intensity of care that is neededEnsure a comprehensive assessment is completed and coordination of allservices provided are provided by phone or face to face contactIn coordination with the Programs Director update policies and procedures.Assure all housing specialist staff have initial and annual Performance Evaluationson file with a signed Job Description.Plans, directs, coordinates, and evaluates client admission process and supportservices of the program.Provide and document supervision at least one (1) time a month or more ifneeded.Participate in staff recruitment, interviewing, hiring, work assignments, orientationAdhere to the program budget, participate in finance reporting and meetings,as requestedProcess progress notes daily and review and approve timesheets and expensereports, monitoring for accuracy and/or fraud and abuseEnsures contract requirements are fulfilled by all team membersEnsure all authorizations are accurate and within correct time frames ofconsumers eligible periodEnsure all progress notes are completed within the twenty-four (24) timePrepare and complete all audit requirements every three (3) monthsComplete monthly Team reports within time guidelinesInterfaces with CARE Hawaii, Inc. committeesIn collaboration with Programs Director assists with recruitment activities asneededWillingly performs other duties/responsibilities as requestedAdditional Duties1. Assist with Quality Improvement development and monitoring2. Participate in team meetings3. Prepare written or electronic reports as requested4. Interfaces with CARE Hawaii, Inc. staff5. Participates in conflict resolution as necessary6. Assess workload and appropriately change work and break times if necessary7.
