Caregivers Home Health (now Elara Caring) is looking for a part-time OT to join our team of talented professionals in the Lawrence and Topeka areas. We offer competitive pay, mileage reimbursement, and the opportunity to work with a caring group of dedicated cliniciansJob Summary:Evaluates, plans and administers comprehensive occupational therapy modalities/regimens to patients in their place of residence for the purpose of developing and restoring function through rehabilitative occupational therapy as prescribed by a physician. Duties and Responsibilities:Performs accurate and comprehensive assessmentsObtains physician orders for certification and recertification of home occupational therapyEnsures that changes in patients' clinical status are communicated to other healthcare team members and Agency office staff verbally and in writing in a timely mannerEnsures that physician orders are communicated to other healthcare team member and Agency staff in a timely, accurate, and thorough mannerCoordinates care planning and delivery with the physician, patient and family, and other healthcare team members and Agency staff to facilitate optimal patient outcomesDelivers occupational therapy modalities appropriate to the patient's clinical status and needs, and in accordance with the standard of care for patient's age and with applicable laws, regulations, and standardsEducates patients, their families/caregivers and other staff as appropriate, in appropriate occupational therapy modalitiesProvides, modifies or discontinues occupational therapy modalities based upon an ongoing assessment/reassessment of the patient's clinical status and in accordance with physician ordersAble to appropriately assess, report, and reassess a patient's painUtilizes appropriate pain management modalities that include, but are not limited to, occupational therapy modalitiesEducates the patient and/or caregiver in effective pain management modalitiesProvides occupational therapy modalities in an environment that optimizes patient safety and reduces the likelihood of medical/healthcare errorsDevelops appropriate home exercise programs tailored to the patient's clinical status, needs and ageSynthesizes data from the physical examination and analyzes data to make clinical judgments regarding patient managementIdentifies risk of injuries or threats to life and health in the patient's environment, i.e. environmental/safety/mobility risks and implements appropriate interventions to reduce those risksAssesses the patient for the need for durable medical equipment as well as other adaptive equipment needsAddresses and supports cultural practices providing such practices do not harm the patient or others, and do not interfere with the planned course of occupational therapyParticipates in the organization's information management programComplies with the Agency's policiesAdheres to all laws, regulations, and standardsCompletes and submits necessary documentation in accordance with Agency policy and applicable regulationsSubmits verification of completed visits in accordance with Agency policyParticipates in interdisciplinary team conferences in accordance with Agency policyMaintains current knowledge of community resources. Identifies and accesses appropriate external and internal resources to meet patient and/or family needs and to facilitate optimal patient outcomes while home care services are being provided, and for transfer and discharge processesProfessional Requirements:Adheres to dress code, appearance is neat and cleanAdheres to legal practice standards, including applicable federal, state, and local regulations and standardsCompletes annual education requirementsMaintains and ensures patient confidentiality at all timesDemonstrates effective time management and organizational skills; provides patient visits as scheduled; completes patient care within the designated timeWears identification while providing patient careAttends annual review and performs departmental in-servicesWorks at maintaining a good rapport and a cooperative working relationship with physicians, departments and staffRepresents the organization in a positive and professional mannerAttends staff meetings on a regular basisResolves personnel concerns at the departmental level, utilizing the grievance process as requiredEnsures compliance with policies and procedures regarding department operations, fire, safety and infection controlArticulates Agency adverse event/incident reporting procedureComplies with all organizational policies regarding ethical business practicesMaintains currency of professional knowledge by participating in continuing education offeringsParticipates in the organization's performance improvement and continuous quality improvement (CQI) activitiesPractices in a manner consistent with the professional and Agency code of conductRepresents the Agency in a positive and professional mannerCommunicates the mission, ethics and goals of the AgencyKey Job/Physical Requirements:Must have a valid OT license that is in good standing with the StateGraduate of an accredited Occupational Therapy curriculumTwo years of work experience preferred. Prior home healthcare experience is preferredCurrent BCLS certificationCurrent health certificate/physical examination and TB testingAbility to relate positively and favorably with patients and staffDemonstrate good oral and written communication along with good documentation skillsUnderstands regulations and standards applicable to home healthcareKnowledge of occupational therapy modalities and scope of occupational therapy practice with the State of practiceFlexible, yet organized, with ability to exercise sound judgementAble to communicate effectively in English both verbally and in writingAdditional languages preferredPhysical Demands:For physical demands of position, including vision, hearing, repetitive motion and environment, please request full job descriptionReasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of the position without compromising patient care.
