Description

This person will help clients and their families make plans for the type of care they'll need, where they could get it, and how to get it in the best possible way.

The successful person at this job will be friendly, encouraging, and solutions-oriented, and make other people comfortable even in troubling times. You would build relationships with your clients and with other professionals in the senior care community.

You'll need to be organized, flexible, and dedicated to ongoing improvement in your role and how you do it. This is a new position, and your enthusiasm for new challenges and welcoming new clients and referral sources will be key. If you're not already familiar with how Medicaid for long-term care works in North Carolina, you will have an opportunity to learn that and help families obtain key government assistance when appropriate.

Responsibilities

•Meet with clients, get to know their situation, and monitor any changes
•Develop a plan to assist the client's care needs
•Coordinate care needs with facilities for clients
•Provide exceptional support to busy attorneys
•Track and enter all time worked daily
•Actively participate in team meetings to improve the efficiency and quality of the Firm's processes
•Look ahead and notice potential issues or breakdowns before they become real

Qualifications

•2-4 years of experience in geriatrics or other approved fields of work. The ideal candidate will have work experience in hospice, nursing home, senior care management, or nursing
•Bachelor's Degree in health care, related social work, human services, or another related field
•You are comfortable using computers, reading online calendars, sending and receiving email, and using Microsoft Office, Client Management Software
•Skills and experience are required, but leadership, personality, character, and integrity are significant considerations
