We are currently in search of an experienced Psychiatric Rehabilitation Direct Service Worker for our Elkton, MD location. The Psychiatric Rehabilitation Direct Service Worker at Kolmac Integrated Behavioral Health and all its subsidiaries ("Kolmac" or the "Company") provides assessment and treatment services that promote recovery, full community integration and improved quality of life for persons who have been diagnosed with a serious and chronic mental health condition that seriously impairs their ability to lead meaningful lives. Direct Service Workers are required to provide collaborative, person directed and individualized services that focuses on helping individuals develop skills and access resources needed to reduce or eliminate the maladaptive behaviors that stem from a mental health diagnosis.

Responsibilities:
  • Conduct an initial assessment to gain an understanding of the clients level of functional impairments, strengths, abilities, needs and preferences
  • Develop an individualized and patient centered treatment plan that describes the goals, objectives, and interventions of the treatment process
  • To implement the delivery of services through onsite (clinic setting) and offsite (home/community) services that includes
  • Teaching and Instruction
  • Encouragement of positive change
  • Skill demonstration and practice
  • Promote individual wellness, self-management, and recovery
  • Assist in the restoration and/or improvement of self-care and social skills
  • Facilitate effective strategies for strengthened independent living and the achievement of self-sufficiency goals
  • Expose individuals to academic, vocational and employment training and resources
  • Facilitate culturally relevant activities directed toward improved social relationships
  • To provide services through face-to-face interactions and telehealth
  • To perform documentation requirements related to treatment planning, monthly summaries, progress notes and other assigned documents
  • To facilitate monthly collaborations with the clients assigned mental health therapist and treatment team
  • Maintain the clinical records and compile documentation required by CCG and governing agencies including the signing of all forms and services
  • Participate in staff meetings, program clinical team meetings, clinical supervision sessions and in-service as required
  • Understand and support the CCG philosophy and treatment modalities.
  • Ensure confidentiality regarding client issues, background, and treatment
  • Exhibit clear, professional, respectful, and effective communication when interfacing with families and external contacts
  • Skill in working independently and implementing time management strategies
  • Exhibit a high degree of ethical conduct, integrity, and confidentiality
  • Ensure client information per HIPAA standards
  • Maintain case files within electronic health record (EHR)
  • Other duties as assigned
Qualifications:
  • At least a high school diploma or equivalent. Associates Degree or higher preferred
  • Previous experience working with emotionally and behaviorally challenged adults or experience working in a recovery-oriented program
  • Must be experienced and proficient in using an electronic health record system
  • Strong interpersonal skills
  • Ability to travel in the local area and perform field work
