Rocky Mountain Vital Care Infusion (RMVC) is a fast-growing infusion pharmacy based in Longmont, CO and Colorado Springs, CO. RMVC is quickly expanding based on the need and demand for high quality customer service and patient care in the infusion pharmacy space. We take great care of our patients, customers, and associates. We are looking for more individuals who share that attitude. RMVC is currently seeking a Home Infusion Pharmacist to join our pharmacy team, in the Longmont, CO area. This role requires high level clinical skills, as well as strong communication skills with the ability to interact professionally with all members of the organization and patients on service. The Home Infusion Pharmacist will be responsible for significant portions of daily pharmacy operations and patient communication. Work Schedule: - PRN/Part Time Essential Duties: Home Infusion Pharmacist will be working in a closed-door setting, while communicating with patients frequently, fostering a trusted patient pharmacist relationship guiding patients to long term health outcomes. The Pharmacist will also need to be able to compound sterile products and will be trained to follow USP 797 guidelines. Building strong one-on-one relationships with your patients while they are recovering or receiving routine treatments, and supporting remote patients on a routine schedule and as needed. Participating in patient evaluation and chart review for acceptance, product selection, device selection and supply management. Collect, interpret, and evaluate all physician medication orders for therapeutic appropriateness, cost effectiveness, accuracy, safety and completeness. Obtains, as necessary, and maintains all required documentation in accordance with federal and state regulatory agencies and policy. Implements policies and procedures for the safe procurement, storage, distribution and use of drugs and biologicals, as well as disposal of drugs. Documents all communication with physicians, nurses, other members of the healthcare team and patients/residents and their family members in the medical record. Qualifications: Graduate of a Bachelor or Doctorate degree in Pharmacy. Pharmacist license in the state of CO is required and be willing to apply for out of state licensure as needed. Home infusion pharmacy experience is preferred. Pay is dependent on the applicants relevant experience. Rocky Mountain Vital Care is an equal opportunity employer.
