Caseworker - Assessor

SUMMARY

It is the responsibility of the Assessor to conduct assessments in order to determine the most appropriate level of care for consumers referred to Office of Senior Services. Some assessments are conducted in the homes of consumers requesting home and community-based services. Others are conducted in advance of a consumer's discharge from a hospital. Still others are conducted in long-term care facilities on consumers in need of a medical assistance grant to cover the cost of their care.

Consumers often have complex medical needs and an ability to learn and comprehend the ramifications of these needs, is essential. There will often be interaction with consulting medical professionals to assist in determining what will best address the needs of medically needy consumers being referred to the agency. It is helpful if a growing understanding of medical terminology develops.

Assessors work remotely and hotel at various sites throughout the county during their work day. They must be able to work independently.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Conducts comprehensive assessment of consumer's needs in order to determine their level of care;
  • Completes required documentation in database with proficiency;
  • Compliance with legally mandated 10 day timeframe for completion of assessment;
  • Reviews assessment determinations with supervisor;
  • Attends trainings and maintains a working knowledge of all available programs and services for which consumers may be eligible;
  • Performs back-up duties, as required, to maintain office functioning. This may include all other direct service roles;
  • To be accountable for their time via outlook calendar;
  • Attend monthly assessment meetings.
QUALIFICATION REQUIREMENTS

  • One year of experience in public or private social work and a bachelor's degree which includes or is supplemented by 12 college-level credit hours in sociology, social work/social welfare, psychology, gerontology or other related social sciences; OR a bachelor's degree with a social work/social welfare major; OR an equivalent combination of experience and training including successful completion of 12 college-level credit hours in sociology, social welfare, psychology, gerontology or other related social sciences.
PHYSICAL DEMANDS

Ability to sit, stand and walk for moderate periods of time. Ability to work in all weather conditions. Occasionally may be required to do Medium Level and/or Heavy Level physical work that would include reaching, walking, pushing, pulling and lifting.

They must be able to drive to various appointments throughout Montgomery County on a regular basis. They must be able to carry a laptop to meet with consumers in order to assess their needs. They must be able to sit for extended periods of time in order to complete extensive case record documentation. Case documentation is done via a computer and proprietary software.
