Overview
This position supports the DCH Care Coordination Department and it's business and clinical processes, with a focus on appropriate utilization of resources, patient transitions across the continuum, appropriate documentation and levels of care. This position is responsible for the management, collection, tracking and trending of data related to Care Coordination housed in several systems. Also, responsible for designing and delivering data analysis from the identified system and reports created. (Midas, Medtech, Tableau, and 3M). Provide support for research and data analysis efforts and assists with special projects for the Health System.
Responsibilities
Manages day to day operations of databases that support the collection and integration of data and information used in tracking daily operations and overall organizational performanceActs as a liaison between the external data sources used to compare DCH performanceValidates internal and external data qualityTransfers data to external data sources as assignedConducts performance analysis reports including trends, projections, external comparisons and correlationsCreates and distributes multiple statistical reports essential to DCH planning and goal achievement
DCH Standards:Maintains performance, patient and employee satisfaction and financial standards as outlined in the performance evaluation.Performs compliance requirements as outlined in the Employee HandbookMust adhere to the DCH Behavioral Standards including creating positive relationships with patients/families, coworkers, colleagues and with self.Requires use of electronic mail, time and attendance software, learning management software and intranet.Must adhere to all DCH Health System policies and procedures.All other duties as assigned.
Qualifications
Bachelors' degree in Business, IT, Marketing or related field required; Preferred experience in healthcare data management with research; Microsoft Office intermediate skills required in Word, Excel, PowerPoint and Outlook. Intermediate Microsoft Office Access, Excel pivot tables, and Publisher skills preferred. Keyboarding experience required. Must be able to work independently, multitask, organize key projects, and prioritize tasks, with emphasis on accuracy and attention to detail. Strong team building skills; Strong oral and written communication skills - electronically, face-to-face and over the phone; Trained focus group moderator preferred. Must be able to read, write legibly, speak, and comprehend English. Must be able to read, write legibly, speak, and comprehend English.
