Job Description The primary purpose of your position is to provide, coordinate, and direct the provision of hospice services according to physicians orders and patient plan of treatment based on hospice policies and procedures, through the application of the nursing process. Duties and Responsibilities Steward the philosophy/mission of PH Healthcare, focusing on human dignity and respectful care. Serve as a member of the Interdisciplinary Team (IDT) and attend meetings. Follow Nursing Standards of PH Healthcare. Demonstrate familiarity with hospice, state, and federal regulations. Implement hospice nursing care in accordance with current rules, regulations, and guidelines. Make written and oral reports/recommendations to the Director of Clinical Services regarding the operation of the nursing service department. Participate in the hospice quality assurance program. Build effective relationships with contracted facility nursing staff to ensure integration of hospice care into facility plans. Document direct patient care services to support billing. Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc. Supervise Hospice Aide care. Supervise CNA schedules. Report occupational exposures in accordance with hospice policies and procedures. RN Case Manager Nursing Functions Coordinate physical care of patients, teaching primarily caregivers, volunteers, and family members. Maintain up?to?date patient records with accurate documentation of problems, plans, actions, and goals. Inform hospice and facility staff of unusual or potentially problematic patient/family issues. Provide appropriate support at time of death. Provide the Director of Clinical Services with information regarding nursing needs. Initiate communication with hospice physicians, attending physicians, other hospice staff, facility staff, and agencies as needed. Maintain regular communication with facility nursing staff and review care at SNF plan meetings. Assign personal care tasks to Hospice Aides per the plan of care. Maintain liaison with patients, families, support departments, and others to plan for needs. Qualifications Must possess a Nursing Degree from an accredited college or university and be licensed as a Registered Nurse in South Carolina. Minimum one year of experience as a nurse in a hospital, long?term care facility, or other related health?care facility is preferred. Hospice experience is also preferred.
