Job Summary and Qualifications

Join our decentralized, clinical pharmacy team at TriStar Centennial Medical Center, where you'll play a vital role in ensuring accurate medication management and patient safety. You'll be responsible for rounding with our medical teams, evaluating patient profiles for clinical intervention, and performing clinical consults for medicine and surgical patients. If you're dedicated to delivering exceptional clinical pharmacy care and supporting our team's goals, we want to hear from you.

Responsibilities:

  • Identify clinical interventions based on patient characteristics, history, and current medication profiles
  • Interact with attending physicians, medical residents, and advanced practice providers to make clinical recommendations to improve patient care
  • Perform pharmacokinetic consults for various medications, including vancomycin and aminoglycosides
  • Complete initial TPN consults and make daily adjustments to TPN regimens
  • Provide patient counseling or education for any necessary medication in preparation for discharge
  • Document clinical interventions effectively
  • Provide patient education when appropriate
  • Identify process improvements for the department and facilitate implementation of solutions
  • Assist with pharmacist training and resident and student education
  • Accurately interpret medication orders and assess patient profiles
  • Prepare, label, distribute, and secure medications
  • Maintain knowledge of hospital policies and regulations
  • Supervise and train pharmacy technician personnel
  • Understand use of automated dispensing processes for pharmacy and nursing personnel
  • Retrieve, analyze, and provide accurate drug information
  • Assist with quality improvement initiatives, formulary decisions, and inventory control
What qualifications you will need:
  • Advanced Cardiac Life Spt must be obtained within 90 days of employment start date
  • Basic Cardiac Life Support must be obtained within 90 days of employment start date
  • TN Pharmacist licensure or eligible
  • Bachelors Degree, or Doctorate Degree
  • Previous clinical hospital experience strongly preferred
  • PGY1 residency strongly preferred
