Primary Duties and Responsibilities The Home Health/Hospice Nurse I (RN) role provides direct patient care in accordance with the Nurse Practice Act to assigned patients. The nurse collaborates with other disciplines of the health care team to ensure the delivery of safe, timely and appropriate quality patient care. Responsibilities include providing physical assessments, nursing interventions, pain management, medication administrations, specimen collection, meeting nutritional needs, teaching and discharge planning of patients assigned. The nurse utilizes the nursing process based on evidence?based best practices to meet patient care needs, evaluates and adjusts the nursing care process as necessary with consultation as needed, and confers with appropriate resource personnel when making ethical/legal/medical dilemma decisions. Performs other duties as assigned. Qualifications Education and Experience Minimum of two (2) years experience in hospice, public health, home care, or hospice nursing. Licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. License Required Current licensure as a Registered Nurse in the State of California. Current Driver's License. Certifications Required Current BLS certification prior to clinical rotation.
