JOB OVERVIEW Job Title: Family Medicine or Internal Medicine Physician Job Type: Permanent Full-Time Location: Hobart, IN Service Setting: Outpatient Schedule: Monday Friday, standard clinic hours On-Call Expectations: 1:10 rotation Start Date: Upon completion of credentialing (90120 days) PATIENT POPULATION & SCOPE Patient Demographics: Adults (Family Medicine may also see Pediatrics and Geriatrics) Average Daily Volume / Census: 2026 patients per day Consults & Follow-Ups: Yes Case Mix/Clinical Focus Areas: Preventive care, chronic disease management, general internal medicine, and family practice FACILITY & TEAM DETAILS EMR System: Epic fully integrated HIMSS Stage VII Specialty Backup: Collegial system physicians and subspecialty support Support Staff: Multidisciplinary team with APPs, nurses, and support staff Leadership & Reporting Structure: Reports to practice Medical Director Reason for Opening: Growth and expansion CREDENTIALING & COMPLIANCE Board Requirements: BC/BE in Family Medicine or Internal Medicine DEA & Certifications: DEA, CSR, BLS required Credentialing Timeline: 90120 days COMPENSATION & PERMANENT BENEFITS Base Salary: Highly competitive (negotiable, MGMA guidelines) Incentive Bonus Structure: RVU-based bonuses Relocation Assistance: Negotiable relocation reimbursement CME Allowance: CME funds available PTO & Holidays: Flexible time off Health & Retirement Benefits: Medical, Dental, and Vision insurance 403(b) with employer match Licensure reimbursement (license, CSR, DEA, dues/subscriptions) Life & Disability Coverage Malpractice Insurance (favorable state laws; details available) REQUIRED SKILLS & QUALIFICATIONS Board Status: BC/BE in Family Medicine or Internal Medicine State License Required: Eligible for Indiana medical license Certifications: DEA, CSR, BLS required Experience: Prior outpatient experience preferred but not required Additional Requirements: Strong team-based care philosophy and EMR proficiency DUTIES & RESPONSIBILITIES Deliver comprehensive outpatient primary care services Provide preventive care, chronic disease management, and acute care visits Participate in call ro
