Care Coordination department has an employment opportunity for a Case Management Assistant assists the social work and case management staff in this fast paced department. Works with insurance companies to obtain authorizations, logs reviews or via the payer portal, arranges transportation for discharged patients, visits patients at bedside or via phone to obtain signatures on documents.

Work Schedule is Mon-Fri & every other weekend 8am-4pm.

Responsibilities:
  • Facilitates the setup of transport for patients transitioning to another level of care.
  • Facilitates the completion of the transport required documents.
  • Supports the care coordination staff in sending referrals for patients to post-acute facility via the software system.
  • Facilitates the process of obtaining and following up on authorizations for patients to transition to rehabilitation facilities.
  • Works with care coordination staff to followup on authorizations and/or determinations that are delayed from the payers.
  • Completes the process of obtaining the IMM (Important Medicare Message) from our applicable patients.
  • Functions as a runner to obtain scripts and deliver equipment such as walker, commodes, etc.
  • Supports the care coordination staff in other tasks that support the discharge of our patients and ensure a smoother transition of care.


Qualifications:

Education:

High school diploma or equivalent required

Associates degree preferred

Work Experience:

Healthcare experience preferred

Required Knowledge and Skills:

Proficient in Microsoft Office, Outlook, Excel
Outstanding interpersonal skills required
