Weekend Rotation.**Coverage Area Option 1** : West Chicago, Winfield, Warrenville, Wheaton, Glen Elen, Glendale Heights, Carol Stream, Medinah. Bloomingdale, Woodale, Lombard, Villa Park, Addison**Coverage Area Option 2** : Westchester, Hillside, Broadview, Bellwood, North Lake, Scheller Park, Elwood Park, Franklin Park, River Grove, Rosemont**The role you’ll contribute:**Evaluates and conducts medically prescribed physical therapy treatment programs.**The value you’ll bring to the team:**• Performs initial and on-going assessments of patient's condition.• Establishes, revises and evaluates a plan of care which is appropriate to problems identified and involves the patient/family.• Performs therapy interventions utilizing standard physical therapy techniques and skills as appropriate for the condition of the patient.• Oversees activities of physical therapy assistants, students and other support persons.Qualifications**The expertise and experiences you’ll need to succeed:****EDUCATION AND EXPERIENCE REQUIRED:**• Bachelor's degree required· Work experience not required**_EDUCATION AND EXPERIENCE PREFERRED:_**· Master's degree preferred.**LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:**• BLS Provider credentialed from the American Heart Association (AHA) obtained prior to hire date or job transfer date.• Drivers License credentialed from the Illinois Department of Motor Vehicles (IL-DMV) obtained prior to hire date or job transfer date.• Rehab Therapist specializing in Physical Therapy (RPT) credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer dateThis facility is an equal opportunity employer and complies with federal, state and local anti-discrimination laws, regulations and ordinances.
