Overview MTM Pharmacist Help Patients Get the Most Out of Their Medications Are you passionate about patient care and looking for a role with strong clinical impact and better work-life balance? Were seeking MTM Pharmacists (Medication Therapy Management) to join our growing team. In this role, youll use your clinical knowledge to optimize therapy, reduce costs, and improve patient outcomes. Responsibilities Conduct comprehensive medication reviews (CMRs) with patients to identify medication-related problems. Provide targeted medication interventions (TMEs) and follow-up to support adherence and safety. Collaborate with prescribers, health plans, and care teams to resolve therapy issues. Document interventions clearly and maintain compliance with MTM program requirements. Educate patients on safe, effective use of their medications and address barriers to adherence. What Youll Bring Education: Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy degree. Licensure: Active and unrestricted pharmacist license in the U.S. Experience: Retail, hospital, ambulatory care, or prior MTM experience is a plusbut all licensed pharmacists are encouraged to apply, including new grads.
