The Care Concierge will often have the single most interactions with our patients and their family members in the community. Patients should know their CCs on a first name basis and create a tremendous relationship with them. CCs perform various administrative duties that empower their care team to operate at the top of their license(s). The patient/family experience should be directly correlated with the strength and success of this role.
