JOB SUMMARY: Works with the Food Service Director, Diet Technician and other dietary personnel to ensure that proper nutritional care is maintained. Communicates and establishes a rapport with other disciplines within the facility. Frequently interact with the residents on a 1:1 basis.
ESSENTIAL DUTIES & RESPONSIBILITIES:
Prepares/oversees an initial nutritional assessment of all admitted residents.
Participates in assuring that menus for regular and therapeutic diets are nutritionally adequate and in accordance with the approved facility diet manual.
Works in conjunction with the diet technician to coordinate: weights, meal pattern, nourishments and meal supplement intake and consistency appropriateness, intake study requirements and results, documentation requirements, transfer/discharge summaries, tube feed reviews. Reviews, evaluates and charts (as needed) pertinent lab values.
Continually counsels and educates the patients, residents, family members, and facility personnel on proper therapeutic and basic nutrition in conjunction with diet technician.
Monitors patient/resident meal, nourishment and supplement intake and consistency appropriateness.
Make appropriate referrals to OT, Speech Therapist, Dentist, Nursing.
Works in conjunction with the Food Service Director to assure that:
Policy and procedures are current and reviewed and reviewed annually
Dietetic education programs are planned to keep the dietary and nursing departments updated and informed as required by Education Department.
The facility diet manual is current and approved.
Temporary and permanent dietary changes are nutritionally adequate.
Attends interdisciplinary meetings as requested.
Reviews minutes of facility and dietary department meetings to keep informed of current events within the facility.
Maintains registration requirements in accordance with New York State.
Provides a copy of registration to the facility upon hire and with subsequent renewal.
Attends various meetings, including weekly comprehensive care planning sessions. Also attends seminars outside the facility that are pertinent to job performance and credentials.
Evaluates diet histories and reviews nourishment and supplement records.

No deficiencies on most recent DOH survey related to nutritional services as determined by root cause analysis.
Facility is better than state and federal benchmarks for weight loss, dehydration and fecal impaction and pressure ulcers
Nourishment cost increase does not exceed the national inflation rate for food.
No deficient practices identified on Quality Review related to the prevention and treatment of pressure ulcers.
Provides one facility-wide nutritional education program
per year.
Achieve a score of 90% or higher on patient satisfaction questionnaire related to dietary expectations.
Achieve a score of 90% or better in the patient expectations questionnaire would you recommend this facility

QUALIFICATIONS:
EDUCATION:
Minimum: Associates degree in Dietetics/Nutrition
Preferred: Bachelor degree in Dietetics/Nutrition
EXPERIENCE:
Minimum: Ten full-time equivalent years of education and supervised experience (1600 hours of experience/one year of credit for every 30 semester hours or education)
Preferred: Completion of your Bachelor Educational program experience requirements.
LICENSURE/CERTIFICATION: Valid NYS Dietician license
