Offering state-of-the-art diagnostic and comprehensive treatment services our hospital is home to a 154-bed patient tower level III trauma center advanced cardiac care center and full-service multidisciplinary cancer center. Our team consists of over 100 board-certified physicians and nearly 600 highly skilled employees to meet our patients medical needs. **Responsibilities**
As a Care Coordinator RN with CHI health, you will coordinate the care and services of patients across the continuum of illness; promote effective utilization; monitor health care resources; and assume a leadership role with the interdisciplinary team to achieve optimal outcomes through the tools of case management. The case manager will perform a wide variety of high-level patient care activities to include admission assessment, screen for appropriateness, and development of a discharge plan. Timely and appropriate discharges will be accomplished by evaluating the patient's post-discharge needs and the provision of services necessary to meet those needs. The case manager must be knowledgeable in community resources and alternate care facilities.

**Qualifications**
**Major Experience/Education:**
Minimum two (2) years of acute hospital clinical
experience with ADN or a Masters degree in Case Management,
or Master's Degree Nursing TX RN License
**Licensure/Certifications:**
Registered Nurse (RN) license
**Pay Range**
$53.00 - $53.00/hour
