Mercy Community have a Full-time permanent position with a 7-day rotational roster, for a Care & Service Manager to join our team in Nudgee.
In this role, you will be responsible for the management of health and clinical care outcomes of the residents as well as implementing, monitoring, and developing the systems and processes to manage and maximise aged care funding (ACFI). The Care and Service Manager is pivotal in ensuring Mercy Community residents and their families experience a seamless high-quality customer journey.
**Responsibilities**
* Oversee person-centred care as part of the service philosophy ensuring the dignity, privacy and confidentiality of residents is respected, and the quality of service is delivered to a high standard
* Support clinical staff to meet performance objectives by providing day-to-day supervision, team meetings, workforce planning, training and support, conducting disciplinary processes and performance appraisals
* Demonstrate compliance to all relevant legislation, regulatory requirements, professional standards, and guidelines including, but not limited to, mandatory reporting
* Maintain and conduct aged care funding (ACFI) assessments to maximise funding and achieve best outcomes while working closely with both management and care staff, ensuring all ACFI submissions are made in a timely manner, with no defaults
* Enter and validate all aged care funding (ACFI) data into the Clinical Management System and monitor expiry dates, annual reviews, and new admissions
* Oversee and ensure infection control principles and procedures are maintained and are in line with current best practice guidelines
* Ensure compliance with case conferences, care plans and reviews involving both the resident and their families/carers
**Skills & Experience**
* Registered Nurse with current AHPRA registration
* Demonstrated industry knowledge and understanding with emphasis on Aged Care Funding (ACFI) revenue targets, Infection Control, Quality Assurance and Accreditation
* Experience managing employees and developing staff skills with a strength in coaching skills and the ability to achieve high levels of team engagement
* Ability to assess, plan, deliver and evaluate care for aged residents as part of a professional nursing and multidisciplinary team
* Demonstrated high level ability to use computer IT systems including electronic Care management System (CMS), MS Word and Excel
* Detailed working knowledge of the Aged Care Quality Standards, the Aged Care regulatory environment including the Aged Care Act 1997
* Ability to obtain positive notice in the NDIS Worker Screening and legislatively required personal history screening as required
* Evidence of an up-to-date influenza vaccination in accordance with the current Public Health Directive for Aged Care Facilities
