Social Worker -PRN | Willow Creek Behavioral Health | Green Bay, Wisconsin
About the Job:
The Social Worker I plans, directs, defines and coordinates individualized and group therapeutic support and services to patients with behavioral problems, psychiatric disorders, post-traumatic stress disorder, substance use disorder and physical and emotional abuse. Conducts individual and group therapy, crisis intervention, case management, and education.

Roles and Responsibilities:

ESSENTIAL FUNCTIONS:

  • Coordinates and plans programs and activities to meet patient's social and emotional needs.
  • Promotes recovery, symptom reduction and the achievement of the highest level of social functioning.
  • Provide crisis intervention and assist families in understanding the implications and complexities of a life situation and its impact on lifestyle.
  • Participates in multidisciplinary treatment team meeting to discuss patient assessment, treatment plan and ongoing discharge plan.
  • Communicates or aids family members in understanding and supporting the patient.
  • Facilitates education, therapeutic groups, support groups and referrals as requested.
  • Assists in the development of effective treatment plans that apply therapeutic principles appropriately while ensuring safety at all times for the patient.
ADDITIONAL RESPONSIBILITIES:
  • Complies with organizational policies, procedures, performance improvement initiatives and maintains regulatory and industry standards.
  • Maintains regular and reliable attendance.
  • Accepts responsibility for professional growth and development of self and attends all mandatory facility in-services and staff development activities as scheduled.
  • Maintains professionalism and confidentiality of facility employees and patients.
  • Performs other duties as assigned.

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
  • Bachelor's degree in Social Work from an accredited college or university required.
  • One or more years' experience working with patient population preferred.
  • Ability to effectively assess, plan and implement therapeutic social work services in a multidisciplinary setting.
  • Considerable knowledge of social work principles and practice, social health and welfare programs and government eligibility for these programs; ability to mobilize and coordinate resources effectively.
LICENSES/DESIGNATIONS/CERTIFICATIONS:
  • Eligible for Licensed Social Worker (LSW) or Certified Social Worker (CSW) licensure as required by the state in which the facility is operating.
  • CPR and de-escalation and restraint certification required (training available upon hire and offered by facility).
  • First aid may be required based on state or facility.
