Are you passionate about helping our senior community? Midland Care is now hiring for a Social Worker at the Topeka PACE Center.

Summary
In this position you will play a vital role in providing compassionate support to individuals and communities in need. The Social Worker will assess participants' unique situations, develop personalized care plans, facilitate access to essential resources, and offer counseling to guide them through various challenges.

Midland Care's Program of All-inclusive Care for the Elderly (PACE) is a not-for-profit health care program that helps people at risk for needing nursing home care who meet a level of care requirement stay in their own homes and communities.

Employment Status: Full-Time|Monday - Friday | 8am to 5pm.

Pay range beginning at $56,000+/year (pay based on experience)

$3,000 Sign-On Bonus

Qualifications

Required: Current unrestricted State of Kansas license to practice as a Licensed Master Social Worker. Must have knowledge and skills with regard to health care coordination, pain/symptom management, crisis intervention and counseling. Must possess valid Kansas Driver's License, in force automobile liability insurance and maintain a Motor Vehicle Record (MVR) that meets organization's insurability standard as defined by Midland policy.
