to create Care Plans in the home setting and complete required follow up visits. Qualifications/Education

: 3 years of RN Experience Experience in completing field evaluations in a home setting, start of care assessments, follow up visits after hospital. Resume with experience in opening home health cases, follow up visits. AlayaCare experience preferred. Responsibilities: Utilize our EMR AlayaCare for nursing notes Perform physical exams and health histories before making critical decisions. Provide health promotion, counseling, and education. Administer medications and other personalized interventions. Coordinate care, in collaboration with a wide array of health care professionals. Document nursing notes regarding patient care daily. Must be able to lift 50lbs. Must be able to stand/work for extended periods Additional responsibilities as required.
