Job Summary:
The Home Care Manager plays a crucial role in overseeing the delivery of high-quality home care services to PACE participants, focusing on the supervision and coordination of nurses and aides. The ideal candidate will have strong leadership skills, extensive experience in home care management, and a commitment to providing compassionate care to older adults.
Duties/Responsibilities:
Provide leadership, guidance, and supervision to the home care team, ensuring adherence to core values, regulations, and standards of care.
Hire and onboard qualified nurses and aides. Provide initial and ongoing training, mentorship, and professional development opportunities to enhance the skills and competencies of the home care team.
Oversee the coordination of home care services for participants, including scheduling nurse and aide visits both routine and on-call coverage, coordinating transportation to medical appointments, and ensuring continuity of care in collaboration with the interdisciplinary team.
Monitor the quality of home care services through regular supervision, performance evaluations, and participant feedback. Identify areas for improvement and implement strategies to enhance the quality and effectiveness of care delivery.
Ensure compliance with PACE regulations, policies, and documentation requirements. Maintain accurate and up-to-date documentation of participant assessments, care plans, and service delivery request in accordance with organizational standards.
Advocate for participants' rights, preferences, and needs within the home care team. Address participant concerns or grievances promptly and effectively, fostering a culture of respect, dignity, and person-centered care.
Collaborate closely with members of the interdisciplinary team, including physicians, nurses, social workers, therapists, and other healthcare professionals, to develop comprehensive care plans and coordinate integrated services for participants.

Required Skills/Abilities:

Proficient with Microsoft Office Suite or related software.
Education, Licenses, Certifications and Experience:
Current licensure as a Registered Nurse (RN)
CPR Certification
Minimum of 5 years of experience in home care management
UCAT III certification, or training within 3 months of hire.
