We are seeking a skilled and compassionate Occupational Therapist to join our Home Health Team.Summary:The Occupational Therapist is responsible for the oversight of OT services ensuring the delivery of therapeutic interventions.Employment Status: Monday-Friday 8am-5pmPay Rate: $40/hr.+ based on experienceEssential Duties & Responsibilities Using all information sources available completes initial assessment, periodic assessments and annual assessments on participants' functional ability and restorative potential. Participates in the IDT communicates changes in the participant's condition and assists with the Plan of Care. Assists in 24-hour participant care delivery. Documents in the participant record in a timely manner Determines participant and family needs related to functional ability. Confers with participant and family to identify participant goals and expectations.
