*Responsibilities*:
* Deliver personalized care to residents based on individual needs and care plans
* Perform nursing assessments and assist in developing personalized care plans
* Administer medications and treatments as prescribed by healthcare professionals
* Monitor residents/clients' health status and promptly address any changes or concerns
* Collaborate with caregivers, families, and healthcare professionals to ensure comprehensive care
* Provide education and support to residents/clients and their families regarding health conditions and care plans
* Maintain accurate and updated documentation of care provided
* Assist Nursing Director in planning, developing, organizing, implementing and directing the Nursing Department.
* Provide supervision for staff
* Handle emergency situations with professionalism and competence
* Assist Management in securing coverage for call offs
* Ensure orders are correctly entered in to medication system through pharmacy provider

*Qualifications*:
* Valid LPN license in Pennsylvania
* Experience in personal care, assisted living, or a similar setting preferred
* Strong knowledge of nursing principles and practices related to personal care
* Compassionate and empathetic approach towards residents/clients
* Excellent communication and interpersonal skills
* Ability to work effectively in a team environment
