To meet regulatory, compliance and quality standards of practice, and to support members of the Administrative/Leadership /Quality Team, the PSDA facilitates multiple multi-faceted operational projects within the agency; requiring the collection, maintenance, aggregation, analysis and subsequent reporting of all data. This role also performs as a stakeholder within the technical support of EHR; facilitating implementation and operational processes in conjunction with all other applicable departments.
Develop and maintain data collection systems for the operational activities of the agency including the QAPI Program, and as directed by the Leadership Team.
Perform data processing, compiling and aggregation of data from EHR telehealth and other databases in order to produce organizational reports as well as reports required per state and regulatory guidelines.
Generate reports and graphics for the purposes of presenting results of all operational activities and QAPI projects.
Participation in task-force related to planning, coordination and implementing of EHR telehealth projects.
Work collaboratively with Leadership team to produce EHR telehealth generated reports, including productivity, scheduling, census, etc., as directed.
Participate in EHR updates and coordinate/communicate EHR system releases to ensure all end users are prepared for updated functionality.
Act as a liaison between end users and EHR support services in conjunction with all other applicable departments.
In conjunction with Leadership team, communicate project status and deliverables; following state and federal Conditions of Participation, regulatory guidelines and agency policies and procedures.
In conjunction with the Quality Manager and Leadership Team, assist in the preparation and maintenance of regulatory surveys and medical reviews.
Create and maintain data tracking/analysis tools, documents and materials related to QAPI, Staff Education, documentation and CMS reporting.
Quantify process variance statistics for gap analysis to deploy quality improvement methods and procedures.
Reconcile monthly services with Finance Department, report to, Leadership Team and external authorities as applicable.
Participate in educational activities related to performance improvement activities and regulatory changes including the development and maintenance of department training materials as needed.
Assists in the management of operational forms including, but not limited to: creating, updating, ordering, printing and stock control, while supporting services of HIM (e.g. scanning and filing).
Works collaboratively with the Operations Manager and Administrator to run EHR generated reports.
Reconcile End of Life Planning documentation between Home Health EHR and NextGen.
Performs data processing and improves data aggregation for HH staff productivity through the use of analytics software.
Report patient data outcomes to Quality and Management Staff for review.
Creates and maintains data tracking/analysis tools, documents and materials related to Home Health staff productivity.
Acts as a liaison between end users and EHR support services. Provides desk support and assists in the resolution of end user technical issues.
Proactively support and practice Home Healths mission, vision and values.
Assist with generating Post-Utilization reports. Evaluate data for trends and report to Administrative Team.

Associates Degree preferred.
Healthcare coordination experience preferred.
Basic understanding of state and federal regulations preferred. Knowledge of EHR systems preferred.
Skilled to mine data from existing systems and manipulate data to produce reports into a usable format to allow management team to assess the productivity and cost-effectiveness of department operations as well as care delivered. Proficient in computer programs (Excel, Word, PowerPoint, Access, and Outlook) in report writing and analysis capacity.
Support management on various projects with great results. Highly detail oriented with good organizational skills.
Goal focused, self-directed and able to work with minimal supervision. Excellent written and verbal communication skills.
Driving not required.
Team player that exhibits excellent follow through and a role model for a high standard of professionalism, work ethics and courtesy.
Positive inter-personal skills with the ability to work independently and with the Administrative-Quality Team.
