Bright Harbor Healthcare provides PACT services to more than 160 people in all of Ocean County. The PACT program is a multidisciplinary team for persons with a serious and persistent mental illness. A team consists of a psychiatrist, advanced practice nurse, nurses, clinicians, co-occurring specialists, vocational specialists, mental health specialists, and peer mental health specialists.

Position Title: PACT II Clinical Specialist
Position Type: Full Time; 40 Hours/Weekly
Benefits Eligible: Yes
Location: Toms River
Department: PACT I

Requirements:

  • Facilitate and complete intake process
  • Complete Bio-psychosocial Assessment within 14 days of admission
  • Manage treatment plan review process, ensuring timeliness and appropriate content
  • Educate staff at weekly Team Treatment Planning meetings on how to develop and engage consumer in treatment planning process
  • Assist consumers with activities of daily living through modeling, direct assistance with ADLs and coaching
Requirements
  • Master's Degree in a Human Services field
  • LAC/LSW/LCAD/LPC preferred
  • Two year's experience working with individuals with serious and persistent mental illness
