Our mission is to deliver equitable and accessible care, fostering healthier lives and stronger communities In collaboration with Physicians, the Advanced Practice Provider (APP) is an Advanced Practice/Certified Nurse Practitioner (APRN, CNP) or Physician Assistant (PA) that provides direct patient to individuals within their scope of practice, licensure, and credentials. This can include preventative, primary care, or chronic care services. An APP evaluates and interprets symptoms to diagnose and manage medical or psychological issues under scope of practice. Essential Duties and Responsibilities: The following description is a general representation of the essential duties and responsibilities of this position * Clinical Practice: Perform patient assessment, collect documents, and analyze data related to patient care needs, signs, and systems. Utilize critical thinking and sound clinical judgement for the assessment, diagnosis, or treatment of patients based on physical examinations, test results, or other relevant information to establish or contribute to plan of care. Order, interpret, and implement diagnostic studies and therapeutic plans that arise. Prescribe drugs or other therapeutic devices within scope of practice and in compliance with applicable laws and policies. * Patient Education: Provide appropriate education to patients or family members to assist in the ongoing care and treatment of the patient. Update and modify plan of care as conditions evolve. Collaborate with community agencies to plan comprehensive care for patients served. Exhibit cultural awareness and sensitivities related to a diverse patient population. Provide consultation to patients and families to assist in prevention of illness and overall maintenance of health. * Compliance: Maintain up to date and accurate patient records. Participate in annual chart review, biannual prescriptive review, regular peer review, or other performance improvement activities as assigned. Develop and maintain problem list, diagnosis, and education plans for patients served. * Staff Education: Serve as preceptor to graduate students within scope of practice. Assist in orientation of new staff. Identify and recommend opportunities for training programs for continued education among peers or other staff members. Act as a resource for other healthcare providers and staff. * Collaboration: Refer patients to other departments, including Behavioral Health, Dentistry, for continuation of care. Provide consultations to other departments upon request. Advocate for expanded services to improve patient care or resources in community * Miscellaneous: Provide occasional on-call coverage. Perform minor diagnostic or surgical procedures within scope of practice after providing patient information related to risks and benefits of procedure. * Other Duties as Assigned Qualifications Qualifications: The requirements listed below are representative of the knowledge, skills, abilities, experience, and education an individual must possess to be able to perform the essential duties and responsibilities successfully. Education, Licensure, Experience: * Nurse Practitioner * Graduate Degree in Nursing (Master's or Doctorate) * Current, Active, Unrestricted license in state of Ohio as APRN or equivalent * Board Certification (AANP or ANCC) as Nurse Practitioner in relevant scope of practice * Physician Assistant * Graduate Degree (Master's or Doctorate) from accredited program * Current, Active, Unrestricted licensure in state of Ohio as Physician's Assistant * NCCPA board Certification * Minimum of 2 years as healthcare practitioner in family medicine, internal medicine or similar clinical setting preferred. * Ability to meet pre-hire and ongoing privileging and credentialing requirements including: * Active NPI Number and DEA License required, or eligible to obtain before providing patient care * Ability to participate in Medicare/Medicaid as provider required, established Medicare/Medicaid number preferred prior to hire but will assist in establishing as needed, * Current CPR/BLS Certification Required * Commitment to serving underserved and diverse populations with cultural competence. Skills and Knowledge: * Demonstrated clinical competence and understanding, including physical assessment, health education, counseling, and overall scope of practice. * Knowledgeable of public health policy and practice, particularly related to community health programs * Strong emotional intelligence skills, including self-awareness, sensitivity, empathy, reliability, and consistency. Able to display appropriate cultural competencies and understanding for a diverse racial, cultural, and ethnic patient population. * Ability to communicate effectively orally and in writing, including the use of electronic communication devices. Ability to speak a relevant second language strongly preferred. * Proficient in use of EPIC EHR/EMR and other clinical software. Essential Physical Demands and Working Environment: * Able to sit, stand, stoop, bend, and walk for extended periods of time. * Occasional lifting of up to 50lbs. * Able to maintain professionalism at all times, including when providing care to challenging or stressful patients. * Able to travel locally between sites.
