Adult Day Program Coordinator Base pay range: $19.00/hr - $20.00/hr Schedule Available: 8:00 AM to 4:30 PM, Monday through Friday. Momentum is currently hiring an Adult Day Program Coordinator to join our team In this dual hybrid role, the staff member will provide transportation for individuals supported at Momentum locations in the San Fernando Valley to and from the Day Program in Thousand Oaks, and will also work at the Day Program in Thousand Oaks. Safely operate Momentum vehicles on surface streets, freeways, highways, and interstates in compliance with DMV and Highway Patrol protocols. Operate vehicle lift for people supported in a safe manner and ensure wheelchairs are secured properly; safety policy requires a 4?point securement for all mobility devices. Assist people attending the program with program activities and community?based activities. Assist people attending the program with communication, feeding, mobility, toileting, hygiene, and other activities of daily living. Engage in program activities including computer class, art class, sensory activities, and gardening. Follow company policies regarding the safe and proper lift and transfer of persons supported. Perform other duties as assigned to meet department and/or business needs. Education and Experience A high school diploma, GED, or equivalent; required. A minimum of two (2) years of related work experience; required. Experience working with individuals with developmental disabilities; preferred. Skills and Knowledge Must be able to read, write, comprehend, and communicate effectively in English. Must have three (3) years driving experience. Ability to maintain confidentiality. Requirements Valid and current proof of legal right to drive in California; required. Must have three (3) years driving experience; required. Current DMV driving record printout; required. Successfully pass all post?offer, pre?employment screening requirements, including criminal background check, drug screening, physical exam, and provide proof of eligibility to work in the United States. Ability to run, push, pull, bend, squat, kneel, walk, stand and sit for long periods of time, and lift up to fifty (50) pounds.
