PRIMARY PURPOSE OF POSITION: Complete and direct comprehensive assessments, development and signing off on individualized plans of care for patients; and direct the care management and coordination activities across the team.

EDUCATION: Master's degree in counseling, social work, psychology, nursing, medical school and/or related field; Hold current licensure in good standing to practice in the State of Maine as either an LCPC, LCPC-C, LCSW, LMSW-cc, LMFT, APRN, PA, MD/DO;. Valid driver's license and current auto insurance required.

EXPERIENCE: Prior experience in social services is a plus.

SCHEDULE OF EMPLOYMENT: May vary based on patients needs. Monday - Friday with after school hours, including evenings; occasional weekend work may be required.

ESSENTIAL FUNCTIONS:
  • Complete admission paperwork (e.g. rights of recipient, consent, etc.)
  • Complete initial and comprehensive assessment and develop an Individual Plan of Care.
  • Establish trusting therapeutic alliance with patients.
  • Diagnose and provide ongoing assessment and treatment of mental health disorders.
  • Deliver services to patients in a variety of settings including office, in-home, community and telehealth.
  • Conduct oneself always following ethical standards, maintaining confidentiality and adhering to HIPAA compliance.
  • Coordinate with network providers/team players involved with patients and their families (e.g. Case managers, psychiatrists, teachers, law enforcement, etc.)
  • Prepare and submit for patient's records, documentation thoroughly describing direct/indirect patient contact withing agency organization timelines.
  • Maintain licensure in good standing, completing all required continuing education requirements.
  • Attend individual and group clinical supervisions and other meetings as required and seek out additional consultation and/or supervision when appropriate.
  • Provide clinical supervision and consultation to assigned staff.
  • Refer patients to other services when necessary.
  • Managing oversight of special projects as assigned.
