POSITION SUMMARY

The Home Health Enrollment Coordinator is responsible for the day-to-day coordination of intake services to home health. This position works under the supervision of the lead enrollment coordinator to coordinate the care of HH patients, new patient referrals and resolve patient problems as they arise.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Serves as Enrollment Coordinator and resource for discharge planners for home health. Communicates directly with referral sources to accept or decline referrals. Consults with clinical directors as appropriate.
  • Screens for appropriateness to accept referral based on availability, patient eligibility/program criteria, location, diagnosis, and patient needs. Requests orders as needed, verifies primary diagnosis and PMD prior to service and delegates record request to HIS staff as appropriate.
  • Coordinates with HH Enrollment Assistant for insurance verification, prior authorization requests, service agreement, and needed documents for admission.
  • Prepares, coordinates, and assigns new HH admissions to clinicians based on staff availability and location, with accurate completion of Referral Central.
  • Assists in the development, implementation, and revision of clinical standards and procedures as needed.
  • Ensure all medications, equipment and medical supplies are ordered in a timely and cost-effective manner prior to admission.
  • Thoroughly documents all clinical coordination activities.
  • Works closely with physicians and other health care providers, offering recommendations related to the provision of health care services.
  • Troubleshoots patient care problems as they arise.
  • Maintains knowledge of current trends in home health, hospice & palliative care.
  • Participates in staff meetings, QI meetings, and other meetings as assigned.
  • Maintains up-to-date knowledge of State and Federal home health, hospice & palliative care regulations.
  • Backup to Lead Enrollment Coordinator and the Enrollment Assistants as needed: managing incoming referrals, data entry of new patient information, creating new patient charts, verifying insurance eligibility, prior auth needs, and creating service agreements for new referrals.
  • Apart of the Adoray Office phone rotation
  • Other duties as assigned.
Requirements

Minimum Qualifications
  • Minimum of one to two years of clinical home care and/or hospice experience preferred.
  • Current Wisconsin MA/LPN/RN license required.
  • ADN required; BSN preferred.
Knowledge, Skills and Abilities

  • Exhibit excellent communication and organizational skills
  • Working knowledge of Microsoft Office (Excel, Word, PowerPoint, Outlook).
  • Ability to manage conflicting priorities and handle multiple tasks/projects concurrently
  • Ability to consistently carry out job responsibilities, able to acknowledge when commitments are not met, and take appropriate steps to fulfill responsibility
  • Ability to clearly and effectively convey information to individuals or groups, including ability to choose the appropriate method or methods of communication
