ChristianaCare's Project Recovery Program is looking to hire Full Time Program Specialist to support the program at the Wilmington Hospital.
The Program Specialist will enhance and improve the quality of patient care at Christiana Care (CC) by providing assessment, referral, consultation, and educational services for individuals with a substance use disorder (SUD). This includes supporting Project Recovery patients and patients receiving SUD Medication Management services.
Work Schedule:
Monday- Friday: 8am- 4:30pm.
Opportunities for a hybrid schedule is available but not guaranteed. Hybrid schedule is contingent on the program's developmental needs.
No weekend/ holidays expected.
Key Responsibilities:
Collaborates with psychiatrists, psychologists, physicians, therapists, community-based services and supportive personnel regarding treatment planning, referral and discharge planning to facilitate transition throughout the continuum of care and progress in treatment.
Assesses needs, supports, barriers, and available community resources to address problems related to social determinates of health.
Coordinates with Project Recovery intake personnel to manage new patient admission processes, working with patients and providers to reduce no show and cancellation rates for first appointments and throughout treatment.
Support the expansion of SUD Medication Management (MM) services to include collaborating with CC practices and community SUD treatment facilities to support their patients with a SUD, scheduling new MM appointments, reduce MM no show rate, and increasing MM retention rates.
Generates and maintains professional contacts with recovery-oriented treatment programs, support groups, medical services and community agencies in order to facilitate appropriate referrals.
Manage discharge process and post-discharge follow up to include reporting all administrative and AMA discharges to DSAMH per new regulatory requirements implemented July 2023.
Performs other related duties as required.

Qualifications
Associate’s Degree in Social Work, Behavioral Sciences or health care related field required.
At least two years’ experience working with patients diagnosed with mental health and/or substance use disorders required.
