DUTIES/RESPONSIBILITIES-Demonstrates appropriate judgment skills to be able to make independent clinical decisions in routine patient care mattersEnsures the safe, productive, and efficient operation of the clinic through timely and attentive administrative and clinical supportDemonstrates skills in phlebotomy, obtaining EKG's, injectable medications including immunizations, and administering IM antibiotics.Performs case management functions; coordinating with internal and external resources to support patientDemonstrates prioritization of patient needs and patient flow, phone triage, provider support, nursing assessment, patient education, patient scheduling, and referralsFormulates goals and an individualized plan of care for at-risk patients that involves the patient as partner and considers the individual needs of the patient in a holistic manner; evaluate and revise as necessaryTriages patient situations and coordinates care with physician. Assist physician with all aspects of patient care including assessment, evaluation and education. Demonstrates critical thinking in the identification of clinical, social, safety, psychological, spiritual and environmental needs for the patient care within an episode of care. Practices solid communication skills, and is able to articulate and translate the patient's condition to other care providers and to negotiate and make recommendations for changes in patient care and office practices. Patient documentation is comprehensive and promotes communication between caregivers. Incorporates national professional organization as well as business unit and health system's goals to improve patient safety, quality and satisfaction. Fosters an environment for open communication and collaborative practice, communicates effectively in oral and written formatsDemonstrates accountability for professional development that improves the quality of professional practice and the quality of patient care. Actively participates in practice-based shared governance, goal setting and supports the change and transition process to improve quality of care and the practice environment. Serves as a highly engaged and full partner on the care team and responds willingly to care team member needs for assistance and partnership. Participates in work that improves patient care and the professional practice environment. Actively participates in department or practice-specific quality improvement efforts. Takes personal responsibility in improving patient satisfaction with the quality of care and service. Utilizes research and evidence-based practice to support improvement in clinical care.Demonstrates knowledge of adult learning principles and applies in teaching of patients, families, students and new staff. Provides detailed and appropriate teaching to patients and families to effectively guide them through the episode of care both in the office setting and via phone/written communication when at home. Supports the development of students, new staff and colleagues. Creates an environment of open dialogue, inquiry and continuous development by asking for feedback and improving practice.Perform other duties, as assigned by manager.Prolonged sitting and/or standing and moving about throughout the workday.Occasional local travel between clinic sites.Daily exposure to potentially biohazardous materials.KNOWLEDGE/SKILLS/ABILITIES-Ability to take initiative appropriate to job duties, be flexible and adapt to change, while working cooperatively as a team memberDevelops and maintains productive working relationships internally and externally by demonstrating accountability for actions, enthusiasm, motivation and commitment to patients and colleagues. Demonstrates an understanding of cultural differences and holds peers accountable for healthy relationships.Demonstrated success in customer service/patient, preferably in a medical care facilityDemonstrated success in managing difficult customer situationsStrong computer skills including Outlook, Word, Excel and PowerPoint preferred.Significant knowledge of medical practices and procedures in a primary care environmentAttends a minimum of one outreach event per calendar yearCommitment to NSCHC faith-based mission to provide care to all who request it, without judgment.QUALIFICATIONS (REQUIRED FOR EMPLOYMENT) - Current Pennsylvania RN licensure requiredCurrent CPR certification child and adult, BLS requiredExperience in outpatient and/or community setting preferredExperience with pediatrics and adults highly desirableAct 33 with renewalAct 34 with renewalAct 73 FBI Clearance with renewalAnnual influenza vaccineMust obtain or provide proof of (within 14 days of hire): PPD, Tdap, Hepatitis B Immunity, MMR Immunity, Varicella ImmunityComplete self-attestation of fitness for duty within 14 days of hire.
