Actively participates in medication safety and quality initiatives. Maintains optimal pharmacy practice through continued growth and development and completes all required CEs for licensure in the state of Montana and those required by BHDH Primary Stroke Certification. Contributes and participates in pharmacy education.Other duties as assigned.Essential Job Functions - Transition of Care Pharmacist:The transitions of care (TOC) pharmacist works closely with an interdisciplinary team of physicians, case managers and nurses to ensure the safe transition of inpatients at discharge. The TOC pharmacist role involves reconciling admission and discharge medications, evaluating appropriateness of discharge therapies, providing medication counseling, assisting patients in obtaining the necessary medications at discharge, and coordinating with receiving facilities to ensure prescription needs are met.
