Nevada Palliative Care is looking for a dynamic LPN to join our team and serve patients in their time of need. This position is a full time benefited position. (We offer free medical coverage).

The Clinical Care Coordinator is responsible for driving the clinical process and communicating patient needs to the the appropriate practitioners. The Clinical Care Coordinator strives for communication excellence by bridging the gap between patients, caregivers, and all members of the care team.

This person builds a trusting relationship with patients and caregivers by understanding their critical needs and triages needs to the care team. The Clinical Care Coordinator plays a vital role and is integral at every hand-off throughout the patient journey.

Responsibilities
  • Develop rapport with patients, caregivers and involved persons in the home or facility
  • Perform screenings based on patient history and needs by obtaining personal medical data, including health history, current medications, reason for the visit, etc. and recording in medical records.
  • Screen patient needs and review charts, labs and self-management goals in pre-visit chart review and schedule needed services prior to visit.
  • Follow-up on all tasks such as; open orders for e-prescribe, medication refills, DME, laboratory, specialist referrals, home health, follow up with patients across care transitions, and assist in entering medications in EHR.
  • Assists in development of patient care plans.
  • Schedules clinical follow up visits throughout the care transition and in the SNF.
  • Manages daily communication between providers and SNF, including issue triage.
  • Ensures provider visits schedules are appropriate and prioritized according to patient needs.
  • Provides basic patient instructions on the performance of routine activities of daily living skills.
  • Educates patient and family on how to access patient portal, view care plans, and use features of patient portal.
  • Follows up to obtain any missing practice forms.
  • Notify care team members when visits are not complete.


Qualifications

  1. Graduate of a LPN program required.
  2. Licensed as a LPN in Nevada required
  3. Hospital/Nursing Home/ Internal Medicine/ Palliative Care experience required. Minimum of 2 years of experience preferred.
  4. Expertise in post acute office practice flow and activities; specialty specific disease processes and conditions
  5. Current BLS Card (CPR)
  6. Proficient in EMR computer applications.
  7. Expert customer service skills that are intuitive to the needs of the patients and staff.
  8. Possesses an ability to maintain good interpersonal relationships with patients, families, coworkers, staff of facilities, and other health team members.
  9. Strong independent decision making skills.
  10. Highly organized.
