Nutritionist - Program ManagerCanaan, CTAbout the Position:The Nutritionist will provide nutritional evaluation, treatment and education for adults with substance use disorders age 18 years and older. The Nutritionist will collaborate with the multidisciplinary team, attend clinical meetings, provide Nutritional consultation and counseling to individuals referred by Medical/Clinical team. He/she will identify appropriate medical nutrition interventions for clients in Detox/Residential Treatment. Nutritionist will hold nutrition group education sessions. Nutritionist will manage all aspects of the nutrition program, administrative tasks, growth initiatives and monitor Key Performance Indicators to maintain high-quality standards.Schedule:Flexible Schedule - Approximately 8-16 hours per weekYour Role:Provide consultation, counseling and ongoing treatment to individuals referred by Medical/Clinical teamAdminister treatments or therapies, such as nutritional needs assessments and nutrition interventions as appropriateProvide recommendations for nutritional and dietary supplements in collaboration with the medical staffOrder/ Review lab work or other diagnostic tests as appropriateInterview clients to document symptoms and health historiesAdvise clients about therapeutic exercise and medical nutritional therapiesWork closely with Food Services Director to consult on overall food choices as well as food as medicine interventions for clientsDocument client histories, including identifying data, chief complaints, illnesses, previous medical or family histories, or psychosocial characteristicsConsult with other health professionals to provide optimal patient care, referring clients to additional health care professionals as necessaryObtain medical records from previous providers for the purpose of client evaluation as neededCoordinate with other departments (Medical/Clinical, Wellness, Food Services) to provide integrated client careCreate, revise and update nutrition curriculum bi-annually or as neededMonitor interventions and keep track of monthly goalsComplete monthly Key Performance Indicators as they relate to nutrition program metrics.Maintain timely and quality documentation in clients electronic medical recordQualifications:Bachelor's degree from a regionally accredited institution with a major course which focused primarily on human nutrition or dietetics.Successfully completed the Registration Examination for Dietitians administered by the Academy of Nutrition and Dietetics and/or Certification Board for Nutrition SpecialistsMaintain an active certification in Dietitian/Nutrition through the state of ConnecticutCompensation:The base rate of pay for this position is $35.00 to $45.00 per hour.
